MyHealthDrīv (pronounced My Health DRIVE) helps participants have an easier time getting healthy by providing: TRANSPORTATION, SOCIAL SUPPORT, and HEALTHCARE NAVIGATION.
Our caring, trained drivers become health partners for the entire duration of each round trip—whether to office visits or other health-related needs. During that time, drivers help MyHealthDrīv participants:
Health-related activities can be stressful and overwhelming to anyone at times. Drivers understand what patients are going through. Our drivers give patients a helping hand, making patients' lives a little easier so they can concentrate on getting better.
CommunityHealth IT, Inc (CommHIT) increases community health, develops workforce, and improves lives by planning & operationalizing complex programs that involve combinations of community, health, & technology, in major cities as well as areas which are rural, remote, or resource-limited. Established in 2011, CommHIT is a OneFlorida Clinical Research Network (OneFlorida+) partner.
In 2014, CommHIT developed the Care Coordination Neighborhood model, which is the evidence-based foundation for Community Health Workers (CHWs) incorporating transport into their daily work with the community members they serve.
CommunityHealth IT's MyHealthDrīv is an integral component of the UFHealth Cancer Center awarded and OneFlorida Research Network supported pilot “Project CONTINUITY: CONnecTing hIgh risk aNd Underserved Individuals To care in the communitY.
For the Utah Transit Authority (UTA), MyHealthDrīv tested and demonstrated the benefit of an e-Voucher system for use with Volunteer Driver and Voucher Programs for health-related trips and mobility management. This effort was funded by the Federal Transit Administration (FTA) Innovative Coordinated Access and Mobility (ICAM) grant program.
